As well as providing personal care, washing, dressing, help with cooking, cleaning, and laundry it also supports clients in reducing isolation with day or night sitting. All Hallows Homecare has a dedicated nurse-led team that delivers specialist packages of comprehensive and continuing care to people living in their own homes. The team works with NHS Continuing Healthcare, Social Workers and other professionals to ensure that people with specialist care needs can choose to stay at home rather than a hospital or care home.

Communication between other professionals is established, for example, GP’s, Specialist Palliative care Nurses, District Nurses, other Specialist Community Nurses, eg. Respiratory, Neurology. Also Community Occupational Therapists, Physiotherapists, Pharmacists, Dieticians; to ensure a seamless and consistent delivery of care. To liaise with a range of health and social care organisations and individuals to develop a clear assessment of Continuing Healthcare requirements and assisting with the integration of clients to homecare.

Bespoke nurse led ‘Specialist Nursing Care’ provided from All Hallows includes:

  • Stoma care
  • Catheter care
  • Epilepsy, Diabetes management
  • Management of tracheostomies
  • Management and change of PEGs
  • Respiratory care
  • Physiotherapy
  • Occupational Therapy
  • Access to speech and language and other professional bodies
  • Palliative care
  • Help and support and training if requested for family members
  • Complex wound management
  • Access to specialist equipment, assistive technology and signposting
  • Weight management, nutrition, hydration support and advice
  • Continence support and advice
  • Post treatments for cellulitis, chest infections, pleurisy and other conditions
  • Help patients with keeping in touch with their community, finding out what activities and social clubs are available.

Because of the unique position of All Hallows, the team has easy access to daycare, outings, respite opportunities and assistance with small adaptations in your home.

Patients requiring Specialist Nursing in their own homes will normally be funded through continuing healthcare. This is an NHS funded package of care for people who have significant ongoing healthcare needs.

Individuals eligible for NHS continuing healthcare, have the right to a Personal Health Budget, this means that money to pay for their care is allocated to them to fund and manage. Personal Health Budgets allow more choice and control over the services and care people receive and can be used to pay for a wider range of items and services. Essentially having a personal health budget allows people and their families to have control over who cares for them and as a health and social homecare provider the team are passionate about this and can assist, hold or manage personal health budgets if their clients would like them too.
